Active Member Optical Benefit
Eligibility
Active members and eligible dependents are entitled to an annual optical benefit by calendar year (benefit is available each change of the year; a full year is not required to pass between benefit distributions.)
How to Claim Benefit
The member must call a particpating Davis Vision Center and schedule an appointment. To find a center, please call Davis at 877-92-DAVIS and use the control number 2942 and the last four digits of the member's social security number as an ID or log onto the Davis Vision Website and use last four digits of the member's social security number as the ID and the first five characters of the member's last name as the password. If the member's last name is less than 5 characters, use the full last name only as the password.
Outline of Benefits
Eye Examination
An eye examination, including dilation as professionally indicated, is covered at a participating network provider with no co-payment.
Spectacle Lenses
One Pair of lenses, including single vision, bifocal, or trifocal lenses, in any prescription range is covered with no co-payment.
Frame
A selection from the Designer Collection on the exclusive Tower display (approximately 300 frames available, including: Stetson, Bill Blass, Cheryl Tiegs, and Elizabeth Arden) is covered with no co-payment. If non-plan frames are selected, a credit will be applied toward a frame purchase from the provider's own selection.
Contact Lenses
The plan covers most popular types of contact lenses including standard, soft, daily-wear (available with no co-payment) or disposable type contact lenses (at either a $20 co-payment for an approximate three month supply or a $40 co-payment for an approximate six month supply.) Once contact lenses are provided, they may not be exchanged for eyeglasses.

- Post-cataract (lenticular) lenses
- Prescription fashion, sun or gradient tinted plastic lenses
- Polycarbonate (impact resistant) lenses for dependent children and monocular patients
Timeframe
Eyeglasses will be sent form the laboratory to the provider generally within two to five business days of receipt of order. More time may be needed when the following items are selected:
- out of stock frames
- glare resistant treatment
- specialized prescriptions
- non-Tower Collection frames
Exclusions
The following exclusions apply:
- medical treatment of eye disease or injury
- vision therapy
- special lens designs or coatings other than those previously described
- services not performed by licensed personnel
- contact lenses and eyeglasses in the same benefit cycle
