Retired Member Optical Benefit
Eligibility
Retired members and spouses are entitled to an optical benefit every two years by calendar year (benefit is available each change of the second year; a full two year is not required to pass between benefit distributions) and eligible dependents are entitled to an annual optical benefit by calendar year.
How to Claim Benefit
To claim the optical benefit, the member or spouse is required to call the SOC (212-964-7500 ext. 1) and request an optical voucher. After determining eligibility, a separate voucher is issued for each family member for whom a voucher is requested. The voucher(s) is mailed to member's address on file. Included with the voucher is a listing of participating optical providers. If no participating provider is conveniently located, the member may mail back the unused voucher along with a paid itemized bill and a reimbursement check will be issued to the member for the worth of the voucher.
Vouchers are valid for six weeks. If a voucher expires unused, the member may mail back the original and indicate that he/she wishes the voucher to be reissued. If the voucher is lost, a request for a new voucher must be received in writing either by mail or by fax: 212-406-3105.
Outline of Benefits
An optical voucher may be used towards an eye exam and materials (glasses or contacts.) Co-payments vary with participating providers and the type of glasses, contacts, etc. chosen.
